My client, a successful Medical Practice near Kirkby is recruiting a Care Navigator Team Leader to supervise their Care Navigating team. Applicants must have previous medical practice background, or worked for the NHS in a similar administration role.Responsibilities:Lead, motivate, and mentor a team of care navigators, providing guidance and support to ensure efficient operations and high-quality service delivery.Support Practice Operations Manager in the co-ordination of annual appraisals for care navigation team staff.Conduct regular performance assessments, provide constructive feedback, and implement strategies to support individual and team growth by training sessions and performance evaluations to monitor progress and identify areas for improvementAttend practice meetings to represent the Care Navigation TeamEncourage a culture of innovation, empowering team members to contribute ideas and solutions for enhancing care navigation services and patient outcomes.Oversee the coordination of patient care activities, ensuring timely and appropriate access to healthcare services and resources.Collaborate with healthcare professionals, including doctors, nurses, and allied healthcare staff, to develop and implement care plans tailored to individual patient needsOversee the induction, training and on-going review for team members and assist in the induction process of all new joiners to the practice care navigation team.Together with Practice Operations Manager, analyse data and generate reports on key performance indicators, providing insights and recommendations for enhancing care navigation services.Collaborate with healthcare professionals, administrative staff, and other departments within the practice to streamline care coordination processes.Skills Required:Must have previous experience working in primary care (NHS)Experience of patient care and working with administrators/care navigators would be desirableStrong administration skills (experience of using EMIS - preferred)Possess good communication and interpersonal skillsHourly rate £13.50ph, 37.5 hours per week - Monday - Friday (rota basis: 8am-4pm or 10.30am-6.30pm), 25 days holiday + bank holidays, pension, onsite parking, wellbeing programme
