Job Description

We are looking to welcome an experienced, qualified General Practice Nurse into our PCN to work with our audit team to identify, assess, plan, develop, implement and evaluate treatment programmes for our patients. Your role will be to look at specific areas of care, such as; Child Health:

  • A reduction in tooth decay from baseline
  • Areduction in childhood obesity from baseline
  • Adolescents:
  • A reduction in STIs and unplanned pregnancies
  • An increase in the number of chlamydia tests completed
  • An increase in the number of teenagers who complete a FAST assessment from baseline
  • A reduction in adolescent hospital admissions for alcohol related episodes from baseline
  • A reduction in the number of adolescent smokers
  • An increase in the number of PHQ9 & GAD7 Assessments from baseline
  • An increase in the number of GPPAQ screening and associated onward referral
  • A decrease in the number of overweight and obese adolescents.#
  • Working Age Population:
  • An increase of diabetes screening for those with risk factors
  • An increase in referrals to NDPP
  • An increase of participants for the NHS Health Check
  • Extended QOF targets for secondary prevention to reduce hospital admissions
  • An increase in participation in national screening programmes and early identification of cancer
  • An increase in FAST screening and associated onward referral if required.
  • Older age population (as above for working age plus):
  • An increase in the number of Rockwood scores saved
  • UTI monitoring and a reduction in admissions
  • An increase in EoL care planning.
  • The main responsibilities of the role include:
  • Undertaking health assessments and performing diagnostic tests;
  • Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met;
  • Healthand well-beingpromotion plus individual person centered plans for patients with/at risk of a long term condition;
  • Managing and treating long-term conditions in line with national and local policies and practice needs;
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments.
  • Prioritising health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care;
  • Supporting and managing health needs of women presenting for family planning and cervical cytology consultations;
  • About you.... If you are an experienced Practice Nurse looking for an exciting new challengethen we would love to hear from you. You will have the following skills and qualifications:
  • A relevant nursing degree and at least 2 years post qualification experience in a General Practice setting;
  • Current NMC registration with an active PIN;
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications;
  • Competence to undertake Asthma, COPD and Diabetic clinics / reviews and hold relevant certificates;
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations and recording spirometry;
  • Compassionate,empatheticapproach anddemonstrablekindness in all aspects of your work as well as valuing the dignity of our patients, relatives, carers and colleagues.
  • Knowledge and application of supporting long-term conditions.
  • Insight and understanding of current issues in nursing and NHS.
