Moving forwards with an integrated workforce an exciting opportunity has arisen for an experienced, innovative Senior Nurse/ District Nurse who is looking for an exciting new challenge.This new way of working offers the opportunity to clinically help shape the development of the Neighbourhood Teams and contribute to key pathways and clinical frameworks, working across the health and social care economy.About youThe successful candidate will be committed to improving patient care from the point of referral, as a Triage Clinician working in a Neighbourhood Team. We have posts available in Woodsley, Meanwood and Holt Park Neighbourhood Teams.This role requires the ability to utilise sound clinical judgement, high levels of communication and organisational skills alongside visible and effective leadership. There will be an expectation that some direct patient care and case management will be incorporated into the role enabling the successful candidates to maintain their clinical knowledge, skills and competences.
