Job Description

Due to the anticipated response to this post it may close before the closing date noted on the advert therefore once you start your application form please complete it immediatelyYou will be an experienced Health Care Support Worker who will support the district nurse in delivering care to patients within the caseload, providing accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies.You will function as an independent part of the district nursing team to provide a quality care outlined in each individual care plan, under the direction of the community charge nurse. You will be required to use your own initiative and judgment in unforeseen circumstances including emergencies, using established procedures and protocols.You should have experience in working with adults in a community health or social care setting and have a SVQ Level 3 in Healthcare or equivalent experience. You should have the bility to work with people and as part of a team, and to work alone in a clinical/homely setting.
