Job Description

The Community Diabetes Nurse works in a skill mix team led by a Band 7 Community Diabetic Nurse Specialist and operates as part of a multi-disciplinary team to deliver holistic care to patients with diabetes who are referred by either primary or secondary care. You will work in conjunction with the referrer to provide diabetic advice in order to optimise diabetes symptom control. You are required to undertake skilled clinical interventions at various stages for individuals living with this chronic disease process. You will assess, plan, implement and evaluate individual programmes of care with reference to patients with diabetes. You will develop care packages for patients who have diabetes and their families/carers in particular for those who require complex diabetes management. You will support patients through difficult life experiences that may affect their diabetes control. Enable patients to optimize their glycaemic control with life-style advice that meets their specific needs. It is essential that you are currently a Registered Adult Nurse with the NMC and that you have a specialist qualification or similar in diabetic nursing. The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.
