Experienced Social Worker - Assessment Team - Medway Council Are you an experienced Social Worker looking for an exciting opportunity to join a dynamic Assessment Team Look no further! You will conduct comprehensive assessments, develop care plans, and coordinate appropriateservices to ensure the best possible outcomes for vulnerable individuals and families.Key Responsibilities:

  • Conduct thorough assessments of children and young people, identifying their needs, risks, and protective factors.
  • Develop and implement care plans, working in partnership with families, professionals, and support networks.
  • Monitor and review the progress and effectiveness of interventions, making adjustments as necessary.
  • Provide expert advice and guidance to colleagues, ensuring the delivery of high-quality social work practice.
  • Collaborate with partner agencies, such as health services, education, and the police, to ensure a coordinated approach to service provision.
  • Maintain accurate and up-to-date records, adhering to relevant legislation, policies, and procedures.
  • Engage in continuous professional development, keeping abreast of the latest research and best practice guidelines.
Requirements:
  • Qualified Social Worker with current registration and a recognized social work qualification.
  • Substantial experience in a social work role, preferably within an Assessment Team.
  • Sound knowledge of relevant legislation, policies, and procedures.
  • Strong assessment and decision-making skills, with a focus on evidence-based practice.
  • Excellent communication and interpersonal skills, enabling effective engagement with children, young people, families, and professionals.
  • Ability to work autonomously and as part of a team, demonstrating resilience and adaptability in a challenging environment.
