Primary Care Network (PCN) Care Coordinator - Early Diagnosis of Cancer and End of Life CareThe post is based with Eastgate Medical Group in Hornsea and Aldbrough which is part of the Yorkshire Coast & Wolds PCN including Leven & Beeford Medical Practice; The Park Surgery, Driffield, and The Medical Centre, DriffieldThe hours available are approximately 30 to 37 per week with a pro rata salary that is comparable to the AFC grade 4 (please note we have not adopted AFC grades at Eastgate Medical Group)This is a new and exciting opportunity to make a difference in the Yorkshire Coast and Wolds Primary Care Network.Are you interested in a unique role which will place you as a key contact to support patients recently diagnosed with cancer and also whilst they receive their treatment You would help our patients to understand their condition and what matters to them and then match the support available locally. Crucially, you will also work with the practices in our PCN to develop and implement best practice which promotes the early diagnosis of cancer and provides high quality end-of-life care.This will mean that as part of the role you would take part in regular MDT meetings, increase early cancer diagnosis rates by promoting national screening programmes, organising educational events and case reviews. Your excellent IT skills will come to the fore through the coding and analysing of data to drive improvement in health outcomes for our patients. As well as being efficient you must also display empathy for both our patients and their carers’, this will come with its challenges as you will often be a cancer patients’ first point of contact. From first point of contact to main point of contact, this unique role will allow you to develop a strong relationship with cancer patients and their carers’.
