Job Description

There are approx 7 newly diagnosed people with MND (pwMND) each year and 25 people living with MND at any one time in the Wirral. This is a life limiting condition that has a devastating impact on the person and their families from diagnosis.This new role is set to pioneer a new service and means of supporting these families.This role is set to be a point of information and support from diagnosis, linking the pwMND and their families to the appropriate services within health care and voluntary sectors. With the intention of enabling them to live fully for as long as possible.The post holder will liaise closely with the MND specialist team and Walton Neurological Centre, the local Wirral MND Assoc team and the Hospice Multi-disciplinary team. They will also coordinate the local MND Multi Professional meetings.The role will sit in the Hospice Wellbeing Team benefiting from the support of manager and colleagues and the educational and developmental approach of the Hospice.
