Job Description

Our Hospice at Home Team support patients and their families to allow the patient to remain in their own home in their last weeks and days of life.The team works closely with the Community Nursing team, palliative care specialists and the wider hospice team to give the best support possible.In this role on day sits you will spend 3 hours per session, in the patients own home providing practical assistance and emotional support or an 11 hour night shift.Experience of working with people with long terms conditions or approaching the end of their life will be an advantage. Hospice at Home HCAs are often present at the patients death.There is supportive line management and an onsite Clinical Educator offering in-house development. We support external education where this is identified in staff development and appraisal process.This is a role where two days are never the same and you have the chance to improve the experience of patients and families when life is most vulnerable.
