Job Description

To provide a responsive, efficient, high quality and equitable service for IBD patients requiring investigation, treatment and monitoring of lower GI symptoms in order to ensure optimum state of health.The overriding purpose is to support the provision of the highest quality patient care through personal actions and continuous improvement.To act as an educational resource to other health professionals caring for patients with IBDand lower GI conditions/disorders.Manage a case load of patients with complex IBD needs maintaining responsibility and accountability for all elements of professional nursing practice.Provide telephone support/advice service for patients, carers and other health care professionals in order to ensure high quality appropriate care and management for patients.Receive and act upon referrals from other health professionals; utilising skills and advanced knowledge base in order to perform clinical examination, arrange appropriate investigations, interpret results and organise appropriate therapy, follow up and referral to other health professionals where necessary.Co-ordinate and run nurse led IBD clinics each week, in addition to managing and reviewing patients on an urgent basis, in order to provide an expert, efficient service and contributing effectively to chronic disease management.Utilise prescribing skills to streamline and optimise care and management of patients withIBD.Manage care for patients receiving innovative therapies who require regular. Co-ordinated, timely treatment requiring day case hospital admission and close monitoring.The IBD CNS will be involved in the provision of Anti TNF services in the trust. After the decision is made to start the treatment, CNS will liaise with the pharmacist to provide either Injection or Infusion. The IBD CNS will infuse Infliximab while maintaining all necessary monitoring during and after infusion to ensure patient safetyThe IBD CNS will be required to plan and organise the IBD Multi-disciplinary monthly meetings. This includes identifying patients who need to be discussed in the MDT, formulating and circulating the list to all concerned within an appropriate timescale. The IBD CNS will be responsible for participating in the discussion, recording the outcomes and provide information to the patient and implementing the outcomes.Work with other health care professionals, and IBD inpatients in order to reduce length of hospital stay, promote timely discharge and prevent/minimise readmission.Development of appropriate competencies utilising the Trust Competency framework.The IBD CNS will be required to present specialist information relating to role and serviceprovision, research projects and inflammatory bowel disease to groups of healthprofessionals e.g. during Gastroenterology meetings, training days and Medical Grandrounds.The IBD CNS will be expected to l e a d the implementation of the IBD registry/Databasefor the Gastroenterology Service and to facilitate the development of planned virtualclinics support research being undertaken within the department.The IBD CNS will work with other IBD Colleagues to develop pilot for IBD CommunityClinics.The IBD CNS will need to impart complex and sensitive information to patients and theircarers regarding IBD, involving diagnosis, treatment options, and advice for living with along term condition.The IBD CNS will establish and maintain their own workload, maintaining responsibility andaccountability for all elements of professional nursing practice.Management of patients receiving therapeutic procedures and infusions.Awareness of the wider healthcare including 18 weeks, RTT, CWT & diagnostic performancetargets.Experience of managing patients with complex long term conditions.Provide clinical leadership and support to other health care professionals with regard to IBD,by maintaining a high profile in the clinical area, acting as a resource and role model.Maintain and improve communication links with primary and secondary care, in order tooptimise the IBD service and contribute to the improvement of cancer waiting times andquality of service provision.Provide comprehensive, evidence based information to patients with IBD and colorectaldisorders, to empower them to make informed decisions with regard to their care andmanagement options, including acting as a patient advocate, when informed discussion maylead to choices being made with regard to care and treatment.Excellent communication and interpersonal skills.Excellent team player.Ability to build excellent working relationships with departmental colleagues and across otherTrust departments to facilitate the IBD service.Ability to empathise and relate to patients with challenging and complex long termlower bowel conditions.The ability to empathise and relate to young patients with challenging and complex long termlower bowel conditions.Must be able to work in a very busy Gastroenterology team, be an excellent team workerwith the robust ability to prioritise own workloadDemonstrate knowledge of Lower GI and colorectal disorders.Experience of running Nurse Led clinics.Evidence of post-registration experience in Gastroenterology nursing, working in an InfusionDay Unit.The IBD CNS will be required to utilise a wide range of communication methods includingwritten reports, GP letters, written communication with patients and other members of themulti-disciplinary team, electronic communication and electronic computerisedpresentations, while at all times ensuring patient confidentiality.For further details / informal visits contact:Name:Leanne SummersellJob title:Specialty Manager Endoscopy and Gastroenterology
