Job Description

About the Service / DepartmentNia House is consists of 3 services, Nia House, Coningham and Sulgrave in the North Cluster. We support people with enduring mental health crisis aged 18+ who have housing and support needs. They may be leaving hospital, registered care, be homeless, threatenedwith homelessness or living in accommodation that is not meeting their identified needs. The services will be also used as a ‘step up or ‘step down from other supported accommodation, where appropriate.About the role

  • Empower a caseload of clients with a range of needs to manage their health and wellbeing, maintain their independence, identify their strengths, and achieve their goals and aspirations, working in partnership with our subcontractors as required to achievethis, under the direction of Management.
  • Clearly articulate the aims and objectives, pathway and service model to people using the service so they can fully participate in and co-produce their support.
  • Empower service users to engage effectively in their liaisons with professionals within the community such as social work teams, housing providers, health professionals, JCP etc.
  • Work flexibly as part of a team to ensure service coverage.
  • Where applicable, develop co-produced group activities, peer support networks and other community-based approaches to empowering service users to achieve their goals and aspirations and to encourage service users to access these opportunities.
  • Take responsibility of your own learning and development.

About you
  • Good communication skills both written and spoken.
  • Good IT skills with a good understanding of MS Excel & Word.
  • Demonstrate a flexible and proactive approach to work.
  • Strong attention to detail.
