Job Description

Community Independence Service Integrated Borough Lead JOB SUMMARY The post holder has responsibility for the effective operational management of the Community Independence Service (CIS) in one of the constituent boroughs i.e. Hammersmith & Fulham, Kensington & Chelsea or Westminster. This role includes responsibility for

  • the management of an integrated health and social care workforce across the service delivery pathways of Rapid Response, Rehabilitation and Reablment.
  • service performance across the pathways, including quality, safety and effectiveness.
  • management of borough operational budget
  • working with head of service, peers, representatives of partner organisations, clinical leads to identify opportunities for service improvement and innovation
  • Location: Hammersmith & FulhamTemporary: Expected to last 3 months with possible extensionHours per week: 37.5 hours per weekSalary: £34 to £39 per hour (Umbrella) or £30 to £34 per hour (PAYE) MAIN RESPONSIBILITIES
  • To be the borough manager for the CIS integrated health and social care team.
  • To ensure effective performance management.
  • To provide leadership and direction to team leaders in the borough, ensuring that safe, compassionate, responsive and effective services are delivered within budget.
  • To work with the CIS head of service and other borough leads in the implementation of service development strategies.
  • To create a culture in which innovative ideas and strategies can flourish and provide vision and inspiration to enable staff to deliver care more effectively.
  • To identify and participate in the tendering for new business opportunities.
  • To deliver robust financial management in line with service, divisional and trust targets, including realistic Cost Improvement Programmes.
  • To ensure services meet quality governance and performance management standards.
  • To represent the CIS integrated borough service at external meetings with stakeholders.
  • The service operates 8.00 - 20.00, 7 days per week. The postholder will be required to participate in a management on call rota.
  • To ensure the effective operational management of services, providing a framework for achieving the commissioned outcomes. This will include the implementation of agreed management structures and clinical networks and the monitoring of CIS protocols, policies and procedures within own borough area.
  • To manage a budget within delegated limits, implementing Cost Improvement Programmes plans as required.
  • To prepare accurate financial information, in liaison with the Finance Department for inclusion within business cases and bids, as and when they arise.
  • To ensure that issues of diversity are fully considered in service provision and are appropriate to the needs of the community and within the Equality Act 2010.
  • To ensure that buildings, service centres and sites are maintained to agreed standards and meet Health and Safety requirements.
  • To implement the Trust’s information/data procedures and participate in the collection and reporting of statistical information.
  • To ensure that service area achieves performance targets as agreed on an ongoing basis.
