Job Description

We are looking to recruit a Qualified Social Worker for Backlog Continuing Health Care Cases within Hackney Council. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.This is a fantastic opportunity to join a stable working environment with experienced leadership, whilst receiving regular supervision and training opportunities.   • Demonstrate experience in developing good working relationships and partnership working with other departments and services within London Borough of Hackney, City & Hackney CCG, Homerton Hospital, and Integrated Independence Team.
  • Able to challenge CHC decisions constructively and positively.
  • Work with an in-depth knowledge of both legal and statutory frameworks relating to social work practice.
  • Undertaking a small caseload of complex CHC assessments and some OOB Hospital referrals, you will also undertake joint visits and review with IIT therapists for those on the IIT D2A Home pathway.
  • Ensure care and support are delivered in a cost efficient manner, identifying networks, local resources, assistive technology, equipment and adaptations etc.
    • To ideal candidate will have:
  • Social Work England registration
  • Eligibility to work in the UK
  • Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
