Senior Specialist Clinical PharmacistLocation- The Wellington Hospital, LondonContract Type- Permanent, Full time, 37.5 hours per weekSalary -Competitive + Benefits (pension, health cover, flexible benefits and excellent career development)About the roleAs a Specialist Clinical Pharmacist, you will be a key member of the Pharmacy Team assisting in the provision and development of a comprehensive, high standard, safe and efficient pharmacy service which consistently meet the needs of all service users withinthe Wellington Hospital.Duties & Responsibilities To provide a safe and patient-focused clinical pharmacy service to wards. This includes undertaking medicines reconciliation, assessing pharmaceutical needs, assessing and facilitating the use of patients own drugs, ensuring appropriate drug supply To ensure high quality care for every patient, every day To participate in ward rounds in order to make pro-active interventions on drug related issues To communicate with the multidisciplinary healthcare team (doctors, nurses, dietitians, physiotherapists, occupational therapists, and ward managers), to intermediate care, general practice support and community pharmacists in the provision ofmedicine related information and assessment of patients treatment To promote effective prescribing across the hospital following formulary, NICE, and any other relevant national or local guidance To act as an Ambassador for pharmacy in providing a consistently high standard of pharmaceutical service to patients in line with corporate and departmental objectives To support in developing methods of improving medicines management to the directorates To assist with the production / updating of local clinical guidelines relevant to the directorates To participate in clinical and departmental audits, measure adherence to local and national guidelines and provide feedback to staffSkills & Experience GPhC registered Member of Royal Pharmaceutical Society of Great Britain (preferable) Relevant hospital experience Additional post graduate clinical qualification e.g. diploma/masters in clinical pharmacy or equivalentIf you wish to find out more about this opportunity, please apply for this role and a member of the team will be in contact shortly.#LI-LC1
