You will be involved in obtaining and documenting medication histories, facilitating and streamlining the discharge process discharge and liaising with GP’s, community pharmacists and the primary care teamYou must be registered with The General Pharmaceutical Council (GPhC) and have an accredited dispensary checking technician qualification (PDCT).You should have substantial post-qualification experience in a hospital setting and excellent communication skills. Experience of working in a ward environment is essential.
