Job Description

Health & Wellbeing Practitioner x 2Location: Midlothian GP Practices, EdinburghSalary: £26,575 FTE, plus generous benefits package, including a £500 welcome paymentHours: Full & Part Time, fixed term basis until end October 2023(with the potential for extension, subject to funding)The RoleThe Organisation are looking for a full time and part time Practitioner to join their Wellbeing team on a fixed term basis until end October 2023 (*with the potential for extension, subject to funding). These posts are located in Midlothian GP Practices.

  • Deliver holistic one-to-one and group based self-management support to people living with long term health condition(s) across Midlothian
  • Work with a team of practitioners to deliver their Midlothian GP contract
  • Practice persistent engagement to help people who struggle to receive support
  • Support volunteers/paid peer supporters who have lived experience of long-term conditions and are supporting the Health and Wellbeing team.
  • Use a monitoring, evaluation and reporting framework to learn and develop practice
  • Develop and maintain links with referrers and services and supports they signpost people
Skills Required
  • Are you a health and social care practitioner with experience of supporting people living with Long Term physical and mental Health Conditions to self-manage
  • Do you cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities
  • Do you believe people have strengths, are resilient and are experts in their own lives
  • Do you thrive working autonomously in the community setting
  • Do you have good solution focused coaching and group facilitation skills
  • Do you possess the self-belief and courage to influence change in the world around you
