Job Description

Health Trainers will assist people eligible for help through the Probation Health Trainer Service in accessing local services and making healthy changes to their lifestyle.Health Trainers will:

  • work as part of a team to support people to improve their health and develop healthier lifestyles
  • offer practical support to help people achieve their own health goals
  • help people find and use the services they need
  • record and monitor their own activity and results
To help others to adopt a healthier lifestyle:
  • Providing clear, up-to-date information to people within their community about health and well-being.
  • Helping people to develop their knowledge and skills about health and well-being by enabling access to local services.
  • Supporting the Health Trainers in delivering awareness sessions.
  • To liaise with staff and other agencies always ensuring confidentiality with particular regard to the Services policies on disclosure of information.
  • Communicate with individuals in a variety of settings about health and wellbeing , promoting key messages around stopping smoking, healthy eating, physical exercise and alcohol & substance misuse
  • To delivery group health promotion workshops.
  • To maintain levels of stock with respect to health promotion information, data collection sheets, etc. in conjunction other Probation Health Service Team members.
  • To contribute to internal and external publicity and promotion in conjunction with other Probation Health Service staff.
  • To ensure compliance with all policies and procedures, and any additional policies and procedures specific to the Probation Health Service.
  • Inform the Probation Health Service Manager and other team members of ways in which your service might be improved.
  • Signpost individuals to other agencies.
  • Collect and report data and information as required by the Probation Health Service Manager and Derbyshire County Council.
  • Understand and comply with all health and safety requirements relating to oneself and to clients.
  • Attend all specified training.
  • Develop the knowledge and skills with regard to health trainer competencies, namely:
  1. Make relationships with communities
    1. Understand and engage with the client groups and their local communities
    2. Work with Offender Managers and other Probation Health Service staff to identify individuals with health issues
    3. Develop and maintain relationships with individuals who are experiencing the greatest inequalities in health
    4. Build up and maintain knowledge of contacts within the community
  2. Communicate with individuals about promoting their health and wellbeing
    1. Provide people with accurate and reliable information about a range of health issues
    2. Signpost individuals to other agencies for information, support and resources
    3. Provide information to individuals about the relationship between behaviours and health.
  3. Enable individuals to change their behaviour to improve their own health and wellbeing
    1. Help individuals identify how their behaviour might affect their health and wellbeing
    2. Help individuals to develop a personal health plan and make the changes they want to make
    3. Support individuals in achieving their personal health plan
    4. Support individuals in maintaining their behaviour change
    5. Help individuals to access and use local services
  4. Manage and organise your own time and activities to support individuals in the community.
    1. Plan own time and activities around the needs of individuals in the community
    2. Respond effectively to referrals
    3. Keep adequate records as agreed
    4. Alert Probation Health Service Manager to any issues in work (including concerns about individuals or work in the community).
    5. Take an active part in developing own knowledge and skills
    6. Seek advice and support as and when appropriate
  • To undertake any other duties within the overall scope of the job as directed by the Probation Health Service Manager.
  • Attend all specified training and achieve the recognised RSPH Understanding Health Improvement Level 2 award. Work closely with your mentor throughout training and development.
  • Provide data and monitoring information as appropriate to role.
  • Work within the standards and policies of Ingeus.
  • Deal with Type 1 Referrals as laid out in the Health Trainer Team Referral Process Document.
  • Empower and support Service Users (SU) by accompanying them to appointments, helping them to complete applications and navigate accommodation pathways by providing advice and guidance
  • Deliver group/1-2-1 workshops/interventions.
  • Provide information, advice and signposting to SUs.
  • Act as Single Point of Contact for frequently used housing providers to assist them in dealing with tenancy issues and/or managing SU behaviour.
  • Develop effective relationships with all relevant agencies; be aware of referral routes and actively promote good partnerships and effective joint working.
  • Develop and maintain positive and professional working relationships with all Ingeus members of staff
  • Actively assist in the implementation of Ingeus policies and procedures.
