Job Description

About the Service / DepartmentFloating Support is an established and dedicated team who support adults within the borough. Staff work closely with local hospitals, care providers, the ICB (Integrated Care Board (ICB), social workers and community professionals to empower older adultsand those with physical health issues to live as independently as possible in their own homes. An aspiration as well as needs-based model ensures that service users are assessed and supported to complete their life goals as well as reducing risk and relapse.About the Role

  • Line manage, lead, motivate and support a team of floating support workers, volunteer administrator, pool and/or in their day to day work.
  • Act as a senior point of contact for the team, responding to complex casework queries and enquiries, attending meetings, building networks and partnerships across Hestia, LBE, Social Services and beyond as appropriate
  • Develop joint working and monitoring protocols with key stakeholders and partners of the service.
  • Manage service performance related to the contract requirements, including analysis of KPIs outcomes and user experience measures.
  • Ensure accurate submission of reports and address any areas of underperformance in line with each contract requirements
  • Develop and implement service annual Business Plans and Equality plans and Coproduction and Volunteering strategy
  • Will ensure that all referrals to the service are acknowledged, assessed, allocated to appropriate support workers, reviewed and closed within agreed time frames
  • Oversee management of the team rota and ensure duty cover
  • Actively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are followed up promptly and in line with procedures
About You
  • Demonstrable experience of managing services that meets the needs and aspirations of people with a range of long-term conditions, health and complex needs and older people.
  • Demonstrable experience of managing dispersed teams across locations including experience of recruitment, induction, supervision, motivation, appraisal, training, discipline and grievance issues
  • Experience of performance monitoring and delivering consistently high standards.
  • Ability and experience of managing service budgets
  • Excellent knowledge of Community Care, Mental health, Health and Safety, Housing and Welfare Benefits Legislation.
  • Knowledge of methods of assessing, formulating, implementing, recording, reviewing and monitoring person-centered support and assessment plans
  • Good knowledge of MS Office systems
  • Skilled at using computer systems including case management systems to create accurate reports and able to assess and audit staff ability to use computer systems to record client data accurately and to a high standard
  • Ability to write & interpret complex reports
  • Excellent level of oral and communication skills and attention to work
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately.
