Morley and District Primary Care Network are recruiting a Health and Wellbeing Coach to join our PCN Team.This is a new and exciting role, and will help shape and form the layout of our local healthcare offer in Morley, Leeds.The suitable candidate should be passionate about making a difference in primary care and enjoy working as part of a multi-disciplinary team across services.The post holder will work as part of a growing team of health care professionals across the seven practices in Morley and District to deliver health and wellbeing interventions at a neighbourhood population level.The role will be based at a variety of locations across the Morley and District PCN area, plus some of the work may be based in Care Home settings over the next few years.
