Job Description

All Hallows neuro specialist centre ( Oakleaf care ) is due to open in the summer of 2020. We are looking for a dynamic person to support the Multidisciplinary team. This is an Opportunity to work in an specialist environment within neurological rehabilitation.JOB DESCRIPTION: Physical Healthcare AssistantREPORTS TO: Associate specialist, Clinical director, Medical DirectorOverall DescriptionTo work as a member of our Multidisciplinary Care Team in maintaining and delivering a high standard of holistic resident care. The physical healthcare assistant will be expected to work to a high level under the supervision of the associate specialist and strictly in accordance with specific guidelines, protocols and policies. The Physical health care assistant will assist the clinical team in the provision of prescribed programmes of patient care. The physical healthcare assistant is employed by The Oakleaf Group and as such is expected to carry out their role at all sites, as and when required.Hours of workThis post requires you to work a minimum of 3 days per week.Duties and responsibilities

  • Wound care
  • Removal of sutures
  • Assist with pressure care and tissue viability - "Waterlow Scores"
  • Venepuncture
  • Performing ECG’s
  • Administer medications
  • Administer adult vaccinations
  • B/P monitoring with the nurses
  • Preparing and maintaining environments and equipment before, during and after resident care interventions
  • Ensure safe storage, rotation and disposal of vaccines
  • Maintain stock of patient advice leaflets and health display leaflets
  • To support with the nutritional needs of the residents, including taking regular weights.
  • Coordinate booking of GP/ hospital appointments
  • Set up a monthly health screening to ensure appropriate blood tests and checks are carried out on residents.
  • Assist and devise Health check audits
  • Assist qualified Nursing staff with Care plans, risk assessment and audits
  • Assist with all resident new admissions to ensure all health checks and initial screening in place including GP, Chiropody, dentistry etc.
  • Undertake other duties appropriate to the level and responsibility of the job

  • Responsible for maintaining a safe environment in accordance with all Health and Safety, Infection Control, COSHH and other Government Legislation.
  • Assist qualified staff in care delivery as directed.
  • Monitoring and ensuring appropriate use of all stocks and supplies.
  • Ensure you are fully aware and operate within all policies and procedures, regularly updating knowledge of any changes.
  • Responsible for the security of all keys, alarms and communication equipment and that these are carried at the directed times.
  • Carry out responsibilities with due regard to the units equal opportunities and diversity policy.
  • Ensure that all duties are carried out to the highest possible standard and in accordance with current quality initiatives within this area of work.
  • To act at all times in a professional manner, as an ambassador of excellence for The Oakleaf Group.
  • Work with the Multidisciplinary team in maintaining the CQC Inspections and the National Service frameworks minimum standards.
  • To be aware of the responsibility of all employees to maintain a safe and healthy environment for all residents, visitors and staff.

  • Administration
  • Ensure accurate and up-to-date notes of all interventions, consultations and treatments are recorded in the residents’ notes.
  • Maintain own personal information and records.
  • Record untoward accidents, incidents and events for residents, colleagues and self.
  • Be up to date and aware of all residents Care plans, Care needs and Risk assessments.
  • Maintain an up to date knowledge of all company policy and procedures.
  • Attend and participate in meetings as required.
  • Communication
  • Communicate effectively with the multidisciplinary team.
  • Assist in discussions and meetings in relation to residents.
  • Communicate to families, relatives and other professionals as directed.
  • Responsible for reporting any perceived concerns relating to residents’ physical or mental health to qualified staff.
  • Maintain resident confidentiality/data protection in line with unit and policy.
  • Attend regular scheduled team meetings.
  • Report/action any maintenance repairs and/or risks.
  • Act on and report any cleaning issues or poor standards.
  • Clinical
  • To work within guidelines of all care plans and risk assessments.
  • Maintain an up-to-date knowledge of resident’s information.
  • Read relevant reports and communication aids.
  • To promote residents choice and equality, and to respect the privacy, dignity, needs and beliefs of patients.
  • To be fully aware of all emergency procedures.
  • To assist qualified staff where required in medical procedures.

  • Attend and maintain an up-to-date knowledge of all mandatory training and other unit in-house training as prescribed.
