Band 6 - Frailty Nurse - Older Adults Health and Wellbeing Service - ThurrockThe role of this job is to support increasingly complex patients across Thurrock within the Older Adults Health and Wellbeing Service. These would be patients in their own homes and in care homes. We carry out comprehensive geriatric assessments and provide management plans which will include advice, guidance and support for individual pathways.NELFT are committed to provide a working environment that promotes Staff Health and Wellbeing encouraging staff to engage in a wide range of different activities to support this. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes.
