Job Description

Are you committed to improving outcomes of vulnerable adults and their carers Are you looking for a challenging role within a dynamic serviceWe are looking for a Social Worker to work in Discharge to Access TeamLocation: CheshirePay rate will be £35/Hr.Your key responsibilities will include:

  • Our aim is to provide a social work service to adults who have been discharged from an Acute Hospital setting or in some Cases stepped up from the community through Discharge to Assess Team and Intermediate Care Team which provide a similar service butat different bases.
  • The post holder may be required to assess patients/ customers in their own homes and other care home settings.
  • Experience of Intermediate Care, Discharge to Assess social work or hospital discharge social work is essential, as well as the ability to manage your time and prioritise workloads. Knowledge of safeguarding is essential.
  • Actively contribute to maintaining quality standards and ensure self and staff development is maintained.
Reed are now offering a joining bonus of up to £250 for Qualified Social Workers. Terms apply.Essential Requirements:
  • SWE Registered
  • Must have at least 3years of post-qualifying experience working with adults in a statutory setting
  • Be a car driver
