Job Description

The Assistant Practitioner will function as part of the multidisciplinary team within the ward/department, to provide a seamless service to patients under the remote supervision of a registered nurse/practitioner. The post holder will continue to develop and undertake a range of delegated tasks and delegated authority and will report to a registered practitioner. The Assistant Practitioner will provide a high standard of general/specific care for an identified client group and will be proficient and competent to work across professional disciplines.This is an exciting opportunity to be included in a developing Colorectal Cancer service with the opportunity to develop and mould the service to meet our patient’s needs.If you are dynamic, excited for change and enthusiastic, then please arrange a visit.As a valued team member you would be developed to meet the following:

  • Bespoke orientation
  • Interact within Multidisciplinary team
  • Attendance of Colorectal MDT
  • Attendance of educational events relevant to service.
  • Manage patient data on Somerset and other relevant databases
  • Review Survivorship and Macmillan Recovery Package for colorectal cancer patients
  • Complete Holistic Needs assessment with patients via phone and face to face in clinic
  • Completion of End of treatment summaries to GP and patient
  • Support Nurse led clinics alongside Colorectal CNS
  • Sign post patients and relatives to other agencies and services as appropriate
  • Patient Advocate
  • Complete Audits
  • Manage Colorectal Support Group and other events relevant to service
  • Take part in service development
  • Manage own workload
