Job Description

** Fixed term contract / secondment opportunity for 2 Years **Are you a professional working in cancer care looking for a new challenge The Norfolk and Norwich University Hospital are working with the Norfolk and Waveney STP to pilot Cancer Care in the Community. The posts are based at our Cromer District Hospital providing an Acute Oncology and Haematology service to the local population. The service will replicate the existing AOHS at the NNUH, working closely together and with the possibility of cross site working. The service will focus on treating chemotherapy / radiotherapy related complications, admission avoidance, symptom control, disease progression, to our North Norfolk patients.This is a two year fixed term pilot to prove the value of an AOHS in multiple localities. As an Assistant Practitioner you would support the Advanced Clinical Practitioners in the care of patients accessing the service. Experience at Band 4 Assistant Practitioner level

  • Able to second check medications
  • Experience of Oncology and Haematology and/or Acute services
  • Good communication skills
  • Work autonomously within Cromer AOHS service as well as working as part of the wider cancer team
  • Ability to assess, care plan and sign post
  • Audit and reporting skills
  • Work closely with all disciplines and external providers
