Job Title - Occupational TherapistBand 6Directorate Central and South ManchesterLocation/Base Chorlton HouseResponsible to Review Team ManagerAccountable to Professional Lead Social Care Professional Lead for Occupational Therapy Professional Nurse Lead Nurse Medications Management teamJob Summary/PurposeThe post holder will be based in Central Manchester but will be required to work across the city andconduct placements visits in other boroughs. The post holder will be responsible for reviewingpackages of care funded by Adult Social Care Mental Health and the Integrated Care Board. They willbe part of a multi-disciplinary team to which they will contribute the skills and expertise unique to theirprofessional group.The post holder will always aim to promote the independence and well-being of people, to assist socialinclusion and valued community presence and enable the widest possible expression of chosenlifestyle. They will work in a Strengths Based and Recovery focused way ensuring the maximisation ofindependence of people living in funded placements and promote step down wherever possible.They will have good working relationships with Manchester City Council Commissioning Team, TheIntegrated Care Board Commissioners, Local providers, the local advocacy service, the ContinuingHealth Care Team, GMMH CMHTs and MCC community teams.If they are an AHMP they will continue to support the AMHP rota. If they are not an AMHP they willhave opportunity to train as one.
