Job Description

Cheshire West Integrated care partnership have the pleasure in offering an innovative and fresh opportunity for a salaried GP to join the REACT /Hospital at Home (Rapid Enhanced Acute Community Team) service supporting people with medical interventions at their own home but with opportunity to work across other GP Surgeries in West Cheshire.This is an exciting opportunity to work collaboratively with other multi-disciplinary teams within primary and secondary care and to expand your portfolio and leadership skills in evolving primary care roles. The role will also be part of a Health Education England (HEE) North West Post CCT fellowship program for 1 year. This will allow the successful applicant to participate in HEE professional and academic development programmes.REACT / Hospital at Home (Clinical sessions-negotiable)Our Hospital at Home service is designed to provide monitoring and treatment at home, which has traditionally been provided in hospital, such as intravenous (IV) therapy. People are referred to this service, either to avoid being admitted to hospital or to enable them to get home from hospital sooner. The post holder will be required to undertake medical management including clinical assessment ensuring safe management of agreed pathways (UTI & Urosepsis, Pneumonia, COPD, AKI, heart failure, cellulitis) within the Community setting. Working closely with primary and secondary care colleagues to ensure safe and effective clinical care is delivered to support admission avoidance and facilitate early discharge where clinically appropriate. The successful applicant will provide medical oversight to the established Nursing Team of Advanced Nurse Practitioners and Nurses delivering high quality care within the community.The post holder will be key in helping to develop the service and clinical pathways going forward.GP Surgery (Clinical sessions- negotiable)There will be flexibility to work in General Practice across Cheshire West. It is envisaged the clinical sessions will be a mixture of planned routine surgeries, including some telephone appointments, participation in a limited number of home visits and administrative tasks such as processing of results and hospital correspondence and inclusion in the rota for occasional duty doctor sessions. There is always the opportunity for clinic support if needed.Health Education England North West (Post CCT fellowship programme) (2 sessions)All post CCT fellows will be invited to attend the higher professional Education (HPE) programme for five days each year. They will also be able to apply for the professional education and development (PED) course. Information will be shared about academic opportunities locally.The fellowship programme will allow for development in the following broad 5 categories1) System leadership-supporting emerging GP leadership with the local healthcare system2) Health equity-using the fellowship programme to reduce health inequalities3) Clinical-help GP’s to develop a clinical special interest.4) Academic-supporting early academic careers5) Educational-stimulating early career GP educators through involvement with undergraduate & post graduate educationExcellent communication skills are required and the ability to prioritise workload is essentialHours and payHours available are flexible in the number of days and hours worked in a week and working patterns. We would encourage applications from full and part time working pattern options. There may be a possibility to cover an on-call rota out of hours. (Saturday and Sunday 8am-8pm)Salary according to BMA pay scales and experience.
