You will have the rewarding experience of providing information, guidance and support to people affected by dementia in Bridgend. You will help them in identifying theirneeds, maintain independence, improve their sense of well-being, and putting them in more control of their lives. This post forms part of the Team working around individuals and their families who are experiencing difficulties with changes to cognitive, emotional,or physical health needs.This new post will work as part of the Integrated Community Network teams in Bridgend. The Network Teams consist of staff across several professions. This includes social work, district nursing, community psychiatric nursing, occupational therapy, physiotherapy,speech and language therapy, dietetics and pharmacy. It aims to provide a seamless and holistic approach to an individual’s care or support needs during times of change or challenge.As a Dementia Adviser, you will:
