City Health Care Partnership CIC are looking for highly motivated, enthusiastic individuals to join the developing team of practitioners in the Hull Urgent Treatment Centre based in Bransholme Health Centre.The post holder will be responsible for the assessment, diagnosis, treatment of urgent, and primary care patients (via telephone, face to face or a digital platform) with presentations including the exacerbation of long-term conditions, undiagnosed undifferentiated illness and minor injuries. You will work collaboratively with partner organisations to ensure that care is seamless and timely to ensure the best possible patient experience. They will also promote exceptional standards of patient care underpinned by the implementation of evidence-based practice, audit and with regard to current available guidelines and research.The post holder will be an experienced clinician who will act within their professional boundaries and will support the delivery of policies and procedures. They will act as a positive role model supporting junior staff whist working flexibly between urgent care and primary care to provide safe clinical expert care. Effective leadership skills will be required and the successful candidate will be one who is motivated, dedicated and enthusiastic.Working as part of an established clinical workforce, you will benefit from a multi-disciplinary team, and be exposed to an extensive range of services and healthcare settings and locations. You will work alongside our newly developed multi-professional clinical model, be supported a network of clinicians to share expertise.Applicants must also have a mentorship qualification and experience. This is a developing role; upon successful completion of the development pathway, there will be an opportunity to progress onto the band 7-8a development role.Post holders will initiate clinical assessment including history-taking, clinical examination, diagnosis, treatment and referral; using a range of advanced skills and expert knowledge for patients of all ages presenting with undifferentiated and undiagnosed conditions. The post holder will be responsible for ensuring that the highest standard of clinical care is provided for patients across CHCP.We provide services 24 hours a day, seven days a week, 365 days a year and therefore shift patterns will reflect this.
