Job Description

An excellent opportunity to develop as a Clinical Pharmacist in General Practice.We are a group of GP Practices in Bath and North East Somerset but you will be employed by our local GP Federation Banes Enhanced Medical Services + (BEMS).Successful candidates will be provided with external support from the Centre for Pharmacy Postgraduate Education.We require pharmacists with good clinical knowledge and self-motivation to develop themselves and support patients within a multi-professional team.You will benefit from:

  • Aims: to co-ordinate patient care working within a primary care setting, keeping patients well for longer and out of hospital by providing long term care in the community. This is a patient facing role and may involve conducting full medication reviews for patients from the following groups – housebound patients, patients in care homes, patients on complex multiple medications and patients with multiple morbidity.Objectives:to improve the quality of life and care for these patients, minimise unnecessary medicine-related hospital admissions, encourage ownership and self-management of conditions and to promote clinically effective prescribing resulting in improved value for the care systems.Access to NHS Pension
