Do you want to be part of a Brand New Stroke service in BarnsleyAn exciting opportunity has arisen to be part of the newly developed Integrated Community Stroke Rehabilitation Team which will encompass a brand new Early Supported Discharge service (ESD) in Barnsley.The Early Supported Discharge service provides care to patients following a stroke in a community setting. It enables patients to continue their rehabilitation therapy at home, with the same intensity and expertise that they would receive in hospital. The Integrated team allows for Stroke specific therapy to be continued as required for a maximum period of 6 months.Situated to the east of the Pennines, Barnsley is a cohesive market town in South Yorkshire located halfway between Leeds and Sheffield.The town has excellent transport links accessed from the M1 motorway and good rail links from surrounding areas as Barnsley rail station is situated on the Hallam and Penistone Lines. Barnsley has a vibrant town centre, and is also served with open countryside on its doorstep.The successful candidate will work within a multi professional team and hold a varied caseload of patients. You will be responsible for the assessment and provision of a specialised programme of rehabilitation, meeting the specific needs and agreed goals of your patients to facilitate their rehabilitation and optimise recovery potential.You will work in close liaison with the wider MDT and other community therapy and nursing areas. You will require excellent communication and time management skills.There is a strong network of allied health professionals across the Trust with clear professional leadership and regular supervision within the profession.
