This post is part of the new Integrated Community Health and Social Care Service within West Dunbartonshire HSCP and will offer the opportunity to work as a Specialist Occupational Therapist with all adult care groups within the integrated health and social work teams with West Dunbartonshire. You will provide direct clinical care; which encompasses occupational therapy assessment, treatment and management of patients within the specialist area of Assessment & Rehabilitation within Community Hospital Discharge Team. You will provide dedicated occupational therapy and ensure the effective co-ordination, delivery, regular re-assessment and review of the Care Plans of those patients who have been referred to the service. This will include a care management function, where appropriate. You will act independently to assess and analyse individual patients to determine their need for occupational therapy intervention within the specialist area of Assessment and Rehabilitation through client-centred practice.
