Job Description

Adults Community Resource Team Social WorkerCaerphillyup to £35 Umbrella companyCaerphilly Social Services are looking to appoint an experienced social worker to join their CRT service. This is an ongoing contract offering up to £35 per hour. The contract is offering an immediate start and offers hybrid working between the office in YstradMynach and the comfort of your own home. The ideal is for someone to be based in the office at least twice a week but as part of induction my client will ask staff to be more office based so that they can support learning etc.The team covers a wide range of hospitals but mostly the Aneurin Bevan hospitals so YYF ( Ystrad Mynach), RGH, The Grange, St Woolos, Neville Hall, YAB (Ebbw Vale)- also other hospitals outside of ABUHB. In addition, potentially the team could cover PrinceCharles Hospital, Merthyrthe RCT hospitals and Cardiff hospitals including UHW, Velindre etc.The main focus of this role is predominantly around hospital discharges and support post discharge as the team follow their patients home for a short term period. Furthermore you may also be required to support other team members as you will be based withinthe CRT / Frailty and so Social Workers are asked to visit with OTs/ Physios etc to provide support around mental capacity and more complex social situations. The main remit for the Social Worker would be mental capacity assessments, long term care placements,CHC etc.To apply for this position you will need to hold a social work qualification and be registered with Social Care Wales.Benefits of working for Hoop:
