Job Description

Up to 36 hours per week – fixed term until 31/03/2021. Would you like to be part of an existing service within Livewell Southwest, helping people in crisis to maintain their independenceWe are looking for a Band 3 Support Worker to join our Rapid Response Care Service, providing a 7 day service across the Tavistock, Ivybridge and Kingsbridge coastal and moorland areas, with some patients living in very rural locations.This is an excellent opportunity for an enthusiastic, forward thinking team player. You will be working a 36 hour week as part of an integrated health and social care team providing care in the community to the local population.The role will be predominantly crisis response care work within a patient’s own home. However it will also include ward and rehabilitation support work with therapists and community nurses.The post holder will be well supported by senior staff, as well as the wider multi-disciplinary team. Your mileage and travel time will be paid for. There are extensive development opportunities and you will receive comprehensive training from a range of health and social care professionals.
