The Practitioner has clinical knowledge in this specialist field and works independently and as part of a multidisciplinary team.As part of the Respiratory team the CNS or AHC will be responsible for the co-ordination and care of patients with respiratory illness to ensure they receive the highest quality care. The post holder will facilitate the discharge of appropriate COPD patients from hospital, advise and assess for home oxygen on discharge and whenever appropriate avoid admission for COPD patients.They will also provide oxygen assessment and review to patients within East Berkshire.The CNS provides specialist clinical advice and support to health care professionals within the Hospital Trusts, Berkshire East Community Health Services (BECHS), and constituent GP practices.They will work with the clinical lead with the development of the respiratory service towards a consistent and integrated approach to respiratory care within East BerkshireWhat’s nextIf you share our passion for people and the dedication to excel, we’d love to hear from you.We are happy to answer any questions that you may have.
