Job Description

The Clinical Commissioning Group lies within the boundaries of the East Riding of Yorkshire covering 1000 square miles of rural, urban and coastal geography and is responsible for the commissioning of healthcare services for the majority, but not all, of the East Riding of Yorkshire population (the Pocklington population are the responsibility of the Vale of York CCG). As such we work closely with our neighbouring CCGs to promote consistency of commissioning with our major provider organisations and with East Riding of Yorkshire Council to ensure that we are jointly commissioning and transforming services to improve the overall health and wellbeing of our population.The CCG consists of 36 GP Practices who have come together to jointly commission services for their patient populations.We aim to use Clinical Commissioning to drive the transformation of local services to better meet the needs of our resident population. This will be achieved whilst continuing to work within the national operational guidance which underpin the wider aspirations, and delivery, of the NHS.Vision and Values The CCG, along with the Health and Wellbeing Board, remains focused upon its vision to provide ‘Better care, more locally, within budget through transformation’ with the underlying premise that ‘Everyone needs to do something different’ to improve health and healthcare, including patients, carers, clinicians, wider public, healthcare professionals and social care professionals.This vision is supported by our organisational values of effective partnership working, ensuring everyone counts, providing quality services, effective leadership and integrity, ensuring we involve and continue to improve in the way we behave as an organisation to achieve our vision.An exciting opportunity has arisen for enthusiastic and experienced Registered Nurse to join the Continuing Healthcare Team to help deliver our vision for Continuing Healthcare in the East Riding of Yorkshire. This is an integrated team with clinicians and social workers, working together.As the Band 6 CHC Clinician you will take responsibility as delegated by the CHC Senior Team for the assessment and transformation of care for existing and potential patients who may be eligible for Continuing Healthcare / Section 117 aftercare.You will support the Band 7 Clinician to case manage a complex caseload of people with varied needs including but not limited to physical disabilities, a wider range of health and social care needs including mental ill health and / or learning disabilities, who require ongoing care and support. Working with a MDT you will assess whether an individual has a primary health need using the Decision Support Tool.There will be close working with residents, carers, colleagues in Local Authorities, the Acute Trusts, the Independent Sector, the Clinical Commissioning Groups and any other relevant groups. This may include specialist learning disability and autism care settings or placements within the NHS / Independent sectorAs a case manager you will promote best practice and be responsible for providing professional advice and guidance to colleagues, promoting good relationships and liaising with colleagues from other disciplines, both externally and internally to the CCG.The ideal candidate will have excellent communication and assessment skills. They will be able to show an excellent approach to problem solving and the ability to prioritise their own workload.
