The tasks and duties of the role will evolve as the service further develops, requiring thesuccessful candidate to be flexible, be adaptable to change and at times use their own initiative to complete tasks.As a Registered Band 6 Clinical Coordinator, you will be working within 2 key areas; Clinical Triage and Home First. Within Clinical Triage you will be responsible for the receipt, organisation, prioritisation, triage and decision making, assessment andonward referral of patients with ongoing rehabilitation and social care needs through effective discharge planning.
