Are you forward thinkingDo you want to be involved in an exciting progressive new ventureWe’re looking for an ambitious, enthusiastic, motivated and experienced ACP (advanced clinical practitioner) who will be required work as an autonomous practitioner identifying, assessing, diagnosing and treating older adults with frailty needs while taking the next steps in developing community services. If you have a strong interest in reablement, working with patients strengths and promoting their autonomy by enabling safe discharge to place of residence to maintain their independence in the community, this could be for you.We are looking to recruit a band 8A ACP (Advanced Clinical Practitioner) to work in the as yet unopened Community Assessment Unit covering predominantly night shifts. Working with a full team of therapy staff/nurses/generic support workers/speech and language and dietetics/mental health workers, we aim to provide patient focused care utilising a strength-based reablement approach.This post is full time (37.5 hours) and will consist of mostly night shifts to cover the 24/7 service.CRT (Community React Team) works in collaboration with NWAS (North West Ambulance Service), primary care and wider health economy in order to reduce hospital admissions.The Community Assessment Unit (CAU) will consist of 21 beds including 9 single rooms. Its primary function will be to operate as an older persons assessment unit, accepting patients for step up/step down care and safe discharge into the community.
