About the Role:HMP New Hall Mother and Baby Unit is a 9 bed unit that offers holistic support for mothers and their babies who are often vulnerable and may have complex emotional and health needs. We work closely with a range of professionals and as part of a team to offerthe best positive outcomes for the mothers on the unit and this is a vital part of our work.As an Family Support Practitioner, youll be:

  • Providing a high quality service to children, young people and their families, based on an assessment of need and is outcomes driven.
  • Delivering a complex and specialist interventions with children, young people or families on a 1:1 basis, group or community setting. This including parenting programmes and group parent sessions.
  • Managing a case load for the one2one work required, but also working as a team to support family contacts and family events, such as family visit days within the prison.
  • Providing lead case/care planning management, including undertaking assessing, reviewing and managing risks.
  • Attending and participating at relevant multi agency meetings, providing a lead role ensuring reflective and analytical practice review.
  • Taking responsibility for day to day resource planning to ensure high quality services are delivered.
  • Establishing and developing effective relationships and channels of communication with children, young people, families, colleagues, community organisations, partner agencies and other external agencies.
What Youll Bring:Our job description for the Family Support Practitioner outlines the requirements of the role but the must-haves are:
  • A minimum of a level 3 qualification in childcare, social work, social care, counselling, youth work or education.
  • Experience of working within a multi-agency social care, early years or health environment and applying safeguarding procedures.
  • Experience of managing cases, in accordance with agreed procedures and completing assessments, planning and reviews to ensure positive outcomes.
  • The ability to keep the child and or young people as the focus and ensure their feelings and wishes are represented in meetings and record keeping.
  • The ability to communicate effectively and engagingly and to maintain independent professional relationships, boundaries.
  • Working knowledge of an outcome based approach to planning, delivering and reviewing services.
