Qualified Social Worker - Hospital AssessmentLocation: South WalesPay rate: £38 per hourWe are looking for experienced and passionate Qualified Social Worker to work in the Adult Hospital Assessment Team based in South Wales . The role has become available to join a committed and motivated Hospital Assessment Team who work hard to deliver excellent standards in a positive working environment. TheHospital Team works in partnership with individuals/ multi professionals to identify what matters to them. This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safeand timely discharge from hospital.This team works alongside a range of professionals within the teams, including colleagues from the health board, therapies, external providers and families. It is an exciting time to join adult social care with friendly, experienced and supportive colleagueswho are dedicated to providing a service that puts citizens at centre of our work.Working in accordance with social care legislation and local government requirements, informed by the values and principles of social care; you will be supporting in the delivery of person centred, safe and effective social care services. Using your professionalknowledge and experience you’ll make assessments and decisions about the most appropriate interventions. Applying your significantSocial Workexperience and excellent communication skills you’ll establish and maintain relationships with serviceusers and other agencies to ensure services meet identified needs.You’ll be committed to ensure positive and sustainable results of your interventions.
