Liberty Trail Homes CIC is a growing organisation based in Nottinghamshire. We provide supported accommodation to adults who are homeless or at risk of homelessness. Many of our residents also suffer from other challenges, such as mental health difficulties,drug and alcohol related issues or have a background of offending.At Liberty Trail we offer a fresh start to individuals who are recovering from hardships in life. We aim to help empower our residents to address their health and wellbeing while preparing them for independent living. We are a small team who work hard toprovide high standards of accommodation and support to those who really need it!Liberty Trail Homes CIC can offer successful applicants a chance to truly make a difference to peoples lives whilst working somewhere where your opinion, expertise and contributions really matter.___________________________________________SUPPORT WORKER ROLESuccessful candidate will be expected to carry a caseload of residents while working with a small team.This role will include duties such as:· Caseload management· Conducting assessments and inductions.· Managing and processing referrals· Feeding back information to management· Signposting and providing support to access external services and partner agencies for support with: Mental health issues, drug and alcohol issues, employment and training opportunities etc.· Establish positive relationships by providing emotional support to residents.· Keep running records and documentation up to date such as risk assessments and case notes.· Work jointly with residents in assessing their needs, preparing, implementing, monitoring and revising Support Plans.· To organise personal workload effectively.Helping residents with:· Applying for benefits and maximising income· Building life skills, including cooking, cleaning and maintaining a home· Understanding and maintaining a tenancy· Personal administration tasks.SUPPORT WORKER SKILLS & EXPERIENCE:Required:· Experience of supporting clients with similar support needs, in a similar role· Health and social care Level 2 or equivalent· Effective communication skills, both written and verbal· A passion for helping others· Empathetic and have good listening skills· Good timekeeping· Confidence to work independently· Great organisational skills· Computer literateDesired:· Experience in supporting those who are homeless, have drug or alcohol issues, or mental health difficulties· Health and Social Care Level 3· Experience of risk management in residential settings· 3 years Experience in housing or social care sector.· Valid driving licence and own car.Liberty Trail Homes CIC has multiple properties, so your role may include meeting residents away from the property and travelling with them to different appointments.The role is subject to a DBS Check, which will be paid for by Liberty.Work remotely· NoBenefits:· Casual dress· Company pension· Sick payDay range:· Monday to FridayPhysical setting:· Sheltered housingShift:· Day shiftEducation:· GCSE or equivalent (preferred)Experience:· Support Environment: 3 years (preferred)Licence/Certification:· NVQ Level 3 Health & Social Care (preferred)
