Job Description

Care Liasion Advisor - Hospital Discharge TeamSutton Coldfield£23,000Job Purpose - SummaryYou will be working closely with hospital discharge teams in a varied role, sourcing care home placements/packages of care for clients with a variety of needs, including complex physical disabilities, challenging behaviour and mental health needs, workingtowards a quick and safe discharge plan, helping families, often during great times of stress.Key Responsibilities/Accountabilities (this may vary from scheme to scheme and is not exhaustive)

  • Sourcing both short term and long-term care home placements/packages of care for clients.
  • Liaising with care homes, care agencies, senior health and social care staff, supporting clients and their families throughout the process; this can be very distressing for them.
  • Sharing updates and acting as the link between services and service users. This is vital to the successful delivery of a high-quality service whilst ensuring discharge happens within set timescales.
  • A large part of the role will entail updating and maintaining client records on our bespoke in-house database.
  • Participating in hospital tracking/stranded/super stranded meetings.
  • Visiting care homes and building relationships with home managers and their teams.
  • Building knowledge of the NHS in order to understand the pressures and importance of timely discharges.
  • Ensure a timely and accurate record, working to KPIs.
  • Compliance with mandatory training.
  • Working across multiple locations with a flexible approach to working hours.
  • Team player who will go above and beyond to help the team, this role can be varied with ever changing processes.
  • Required to use your own car, insurance to include business use.
Scope and Scale of JobOccasionally required to travel to care homes, in some cases transporting families to and from visits.PERSON PROFILEEducation
  • Ideally 5 GCSEs A-C to include Grade C English and Maths and/or A Level, BTEC, Diploma, or Degree.
Experience
  • Having worked within the healthcare environment is desirable, but not essential.
  • Delivered a high standard of customer care.
  • Administrative skills.
  • Worked within a fast paced/challenging environment.
  • A proven history of achieving KPIs and working to strict deadlines.
Knowledge
  • Microsoft Office (Word, Excel, Outlook, databases).
  • Understanding of confidentiality, data protection and safeguarding.
  • Working to protocols and procedures.
  • Full Driving Licence.
