Key Responsibilities Closely partnering with our discharge coordinators, you will ensure that admissions and discharges are seamless, and all clinical assessments are completed within 24 hours of referrals, to minimise bed blocking within local hospitals. In particular, youwill make certain that referred patients are suitable for our rehabilitation programme and ensure that colleagues at the homes are fully briefed on their needs. At the same time, you will oversee the implementation of care plans, making certain that we deliverthe highest standards of clinical support and outstanding patient-centred services. Skills & Experience Experienced in continual assessment within hospital and care settings, you will be fully familiar with nursing tools such as Waterlow and MUST. You will be skilled in writing care plans, capable of supervising colleagues and ready to reshape working practices.Finally, familiarity with dementia care would be beneficial, as would a background within a rehabilitation and step down facility.
