Reablement Support Worker, Band 4Hammersmith & Fulham, Hounslow & EalingAre you looking for your next challengeWe are looking for exceptional individuals to support and develop our newly commissioned Reablement Service. This is a rare opportunity to be involved of the creation of a service that meets the needs of our service-users.The post holder will benefit from a varied and diverse caseload, working with individuals and stakeholders to ensure a smooth transition and safe discharge back into the community.The post holder will work closely with the Inpatient Occupational Therapy team and Crisis Team, as well as community colleagues and the wider MDT.This role will offer an array of learning opportunities enabling you to expand your experience and skills base.
