Job Opportunity: Transitions Social WorkerAbout Us: Are you a dedicated and passionate Social Worker with a commitment to empowering Care Experienced young people or adults Join Future First, the esteemed Leaving Care Service for Wandsworth Childrens Services. Our service, withan Ofsted rating of Good, is a beacon of excellence in supporting Care Experienced young people aged 16-25, offering a dynamic and multi-disciplinary approach to ensure brighter futures for our young people.Position: Transitions Social WorkerSalary: Up to £49,503 per annumContract: Permanent, Full-Time (36 hours per week)Location: Wandsworth, LondonOfsted Rating: GoodAbout The Role:Experience: We are seeking a Social Worker with a proven track record of improving outcomes for Care Experienced young people facing challenging circumstances. Your role will involve providing vital support during their transitions intoadulthood.Regulatory Compliance: Conduct Care Act Assessments under the Care Act 2014, ensuring accurate evaluation of care and support needs to enhance independence. Oversee the journey from assessment to procurement and establishment of Adult SocialCare services, ensuring seamless transitions.Multi-disciplinary Service: Collaborate with a team of Personal Advisors within the Leaving Care Service to develop innovative support packages. Serve as the lead officer for Adults Safeguarding work related to young people.Performance Management: Responsible for meeting weekly performance targets and positively impacting the lives of young people, reporting to Service Managers for both Leaving Care and Preparing for Adulthood.Hybrid Working: Embrace a flexible, young person-focused approach to work. While youll be based at our office near Clapham Junction station for at least one week per month as part of a duty rota, we encourage regular visits to the Hub.Qualifications and Skills:

  • Qualified Social Worker registered with Social Work England.
  • Proficiency in Trauma Informed and Systematic Practice, with an understanding of key issues faced by Care Experienced young people.
  • Familiarity with learning difficulties and adult mental health systems and networks.
  • A commitment to supporting Care Experienced young people with excellent communication skills and an anti-discriminatory approach.
  • Experience in producing high-quality assessments and reports.
