Main Purpose of PostTo work alongside Powys residents to facilitate their safe and timely transfer from hospital in line with Welsh Government hospital discharge service requirements.Through strengths-based assessment and care and support planning, takethe time to understand what matters to Powys residents and work alongside them, and partners to organise the support required to maximise their recovery and recuperation in a community setting.To identify obstacles to timely transfer and to work alongsidesystem partners and the Hospital Social Work Team Manager to unblock these obstacles so that Powys residents can return home from hospital without unnecessary delays.To act as the main contact for Powys Adult Services with inpatient acute care in Herefordand to be co-located as part of the existing Hereford integrated team to ensure safe and effective transfer of Powys patients back into the community.To contribute to the statutory obligations under the Mental Capacity Act (2005), the Mental Health Act (1983),the Mental Health Wales Measure (2010) and the Social Services and Wellbeing (Wales) Act (2014).
