Social Worker - Older Adult Mental Health Team Location: Slough Salary: Up to £44,470 per annumAre you a dedicated and seasoned Social Worker looking for a chance to contribute to the Older Adult Mental Health Team at Slough Council Join us in making a tangible impact on the lives of older individuals within our community.Responsibilities:Perform thorough evaluations of the mental health and social care requirements of older individuals in the community. Create and execute personalized care plans in collaboration with service users, their families, and fellow professionals, ensuring a comprehensiveapproach to care. Offer therapeutic interventions and assistance to enhance the mental health and overall well-being of older adults, empowering them to maintain their independence and quality of life. Forge close partnerships with allied agencies, includinghealthcare services and voluntary organizations, to synchronize support and services for older adults with intricate needs. Advocate for the rights and concerns of older adults, ensuring their voices are acknowledged and valued in decision-making processes.Maintain precise and current documentation in compliance with statutory mandates and council protocols. Participate in team gatherings, case discussions, and training sessions to enrich professional growth and service provision.Requirements:Qualified Social Worker registered with Social Work England. Considerable experience in mental health services, preferably focusing on older adults. Thorough comprehension of the Mental Capacity Act, Care Act, and other pertinent legislation and policiesgoverning social work practice. Proficient in assessment and intervention techniques, with the capability to devise and execute effective care plans. Exceptional communication and interpersonal skills, adept at engaging with service users, families, and interdisciplinaryteams. Dedication to advancing equality, diversity, and inclusion in service delivery. Capacity to collaborate effectively within a multidisciplinary team and autonomously as needed.
