Care CoordinatorOur client, who are a fast growing domiciliary care services provider with a reputation for providing high quality care. Due to continued expansion, they are seeking an experienced and highly motivated care coordinator to join a highly driven and fun team to be a part of the further growth of the business.Your purpose as the Care coordinator is to ensure effective coordination of care allocation and delivery by working in partnership with the Service Managers and Field Care Supervisors.Key Features of the role:- To ensure that rotas are communicated to care and support staff in a timely manner and ensure that rota queries are dealt with promptly- To work as part of an effective team in implementing and evaluating the aims, objectives, policies and procedures of the company to ensure an efficient and effective service delivery.- To assist in the smooth running of the office and its multi-disciplined service activities.- Support the Registered Manager to organize the day-to-day running of the service.- Assist with the initial assessments of new services.- Assist with the creation of effective written Care Plans; examine the written records of medication administration, finance and daily log notes kept by Staff as required by the Registered Manager.- To produce detailed work schedules for the Care Workers and providing both written and verbal instructions/guidance to them.- To carry out any other tasks required by the Registered Manager that are commensurate with the level of responsibility.Please apply now for immediate consideration and to gain further information about the role. In order to be considered for this role, you must have a strong administration/co-ordination background and be prepared to work 5 days in the week, which may include rotad weekends (not every wekeend).
