Job Description

Support CoordinatorThis is an exciting opportunity to work with stroke survivors and their families to support them following stroke. Were looking for an innovative, passionate professional, whose values match our own, can think independently and is opento a coaching style of management, to join our Stroke Recovery Service based in Thurrock.Position: Support CoordinatorLocation: Homebased in Thurrock, with regular travel across the service areaHours: Part-time, 28 hours per weekSalary: Circa £19,496 per annumper annum (FTE £24,370 per annum pro rata)Contract: Permanent. Please let us know if this will present any challenges when you email your applicationThe RoleTheservice aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.The Support Coordinator will: Support stroke survivorsand their carers into the community Provide personalised information, advice and support Support clients to make informed lifestyle changes which will help them to prevent further strokes To run an effective service for stroke survivors and carers, ensuringthat confidential and accurate records are kept on our CRM data baseAbout YouThe Support Coordinator will have experience/background in: Experience of using technology and IT systems Experience of providing person centred support to vulnerable people A caringprofession ideally with experience of supporting people with disabilities Identifying, referring and signposting clients to local support resourcesThis role is home based, but extensive travel is required across the local area and occasionally further afield.Theuse of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the roleTo fulfil the requirements of the role you must abode in the U.K and have the right to work in the U.K.About the OrganisationWhen strokestrikes, part of your brain shuts down.
