Job Description

About the team:Working as part of our multi-disciplinary team you will be expected to offer assessments and interventions to our client group of older adults aged 65 and over who have mental health needs. And for people under 65 years old with a diagnosis of dementia.About the role:An exciting opportunity has arisen in Croydon CMHT within the Mental Health of Psychological Medicine and Older Adults Operational DirectorateYou will manage a caseload of complex clients as well as support our duty and triage function.The post holder will be expected to work both autonomously and as part of the team, and be able to formulate flexible, robust and holistic care plans with clients and/or carers that aim to guide service users towards reaching an optimum level of functioning and support them to maintain good mental health.The post holder will undertake the responsibilities of CPN Care Coordinator as part of the Care Program Approach for a caseload of clients, working with carers where applicable. The post holder will work within an experienced team comprised of CPNs, Psychiatrists, Occupational Therapists, Clinical Psychologists and Social Workers.The post holder will work within the model of recovery led practice and will facilitate user involvement in all aspects of service delivery.What this post can offer:
