Rapid Response Advanced Primary Care Nurse, Band 7PermanentFull Time: 37.50 hours per week We are looking to appoint a committed, highly motivated, enthusiastic and skilled Band 7 Advanced Primary Care Nurse to our dynamic, integrated multi- disciplinary Rapid Response Team. Please note this post is for a Qualified Adult Registered Nurse (RNA,RN1, RN2).This is an exciting opportunity to join and develop our new integrated collaborative working between Oxleas NHS Foundation Trust and Bexley Adult Social Services (Bexley Care). Would you like to help us continue to improve our provision of health, socialcare and community support to the residents of Bexley.The Integrated rapid response team has been operational since 1st August 2013 and is a multi- professional team consisting of Nurses, Occupational Therapists, Physiotherapists and Social Workers. The team aims to avoid unnecessary presentation to hospitaland residential care, by supporting community patients who are in crisis and to prevent direct hospital admission from Accident and Emergency and other areas such as Acute Medical and Clinical Decision Units.The team provide holistic health and social care assessments for people over the age 18years, who are registered with a Bexley GP and live in the Borough of Bexley. The team is based at Queen Elizabeth Hospital Woolwich and operates 7 days a week, on anshift basis between the hours of 8am & 8pm, providing rapid response to vulnerable people in the community.The team act as a liaison between acute and community settings, by identifying the most appropriate onward referrals and initiating integrated care pathways, ensuring seamless provision of care and on- going case management. Referrals are received from Healthand Social Care professionals, Medical Teams, General Practitioners and Ambulance Services.The Rapid Response team is able to provide community follow up and support and review within a predetermined of time frame.The post holder will lead and develop the rapid response Nurses, support in the leadership and development of the Rapid Response Team, deputise in the absence of the Manager for the clinical management of the care of patients referred to the team. The postholder will assist with and directly triage urgent patient referrals with varied and complex needs in acute hospital and community settings, acting as an expert nurse clinical resource to Rapid Response Team members by facilitating high quality evidence basedpractise.The post holder may be required to work across hospital sites on a planned basis and in community when responding to urgent community referrals. Post holder will undertake delegated line management of junior staff, both informal and formal basis.
